Health Care Surrogates

A Health Care Surrogate Designation is a document by which you name someone as your Health Care Surrogate or Proxy for health care decision making, should you be unable to make these decisions for yourself in the future. These can be useful whether you encounter a temporary or permanent situation in which you are incapable of making your own informed medical decisions. Health care surrogate designations can also be accomplished through a Health Care Power of Attorney, or incorporated within a general durable power of attorney which covers not only health care decision making but also all financial matters.
